New Patient Registration
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill. We also require one form of photographic ID. If, for whatever reason, you are unable to provide ID or proof of address you can still be registered for and receive care. Please follow the link below for further information.
How to register with a GP practice
You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Medical treatment is available from the date of registration. Please contact reception for further information.
Personal Details Update
Help us to keep our records up to date by informing us of any changes to yours, or any member of your households, personal details. So if you get married and change your name, move address or get a new phone number then please let us know about it.
You can either telephone the surgery on 01296-713434, call in and tell us about it, fax it to us on 01296-715439 or send an e-mail to firstname.lastname@example.org.
